We are now in network with Horizon Blue Cross Blue Shield Commercial Insurance and Medicare! All other insurances are considered out of network and cash pay. Therefore, we request that you make payment at the time of the appointment or service. Payments can be made via debit or credit, and a valid credit card must remain on file for future payments for services and insurance copays. If you have out of network benefits, an invoice can be provided for you to submit to your insurance company. Rest Assured Mental Health Wellness is not responsible for submitting any out of network insurance claims. When scheduling an appointment, you are acknowledging the services rendered will be paid in full regardless of coverage. Please contact the office for fee schedule. 

Please Note: There will be a non-refundable fee of $110 for initial appointments and $75 dollars for follow up appointments not canceled within 48 hours of scheduled appointment.
