We are now in network with Horizon Blue Cross Blue Shield Commercial Insurance, Medicare, Cigna, Aetna, and United Healthcare! All other insurances are considered out-of-network and self-pay. Therefore, we request that you make payment at the time of the appointment or service. Payments can be made via debit or credit, and a valid credit card must remain on file for future payments for services and insurance copays. If you have out-of-network benefits, an invoice can be provided for you to submit to your insurance company. Rest Assured Mental Health Wellness is not responsible for submitting any out-of-network insurance claims. When scheduling an appointment, you acknowledge the services rendered will be paid for in full regardless of coverage. Please contact the office for the self-pay fee schedule.

Please Note: There will be a non-refundable fee of $110 for initial appointments and $75 dollars for follow up appointments not canceled within 48 hours of scheduled appointment.
